If a conditional offer of employment is made, you are required to submit to a pre-employment drug test. Vaccine Requirement:  All employees must receive their first or single-dose COVID-19 vaccination and present proof of vaccination to the HR Department no later than November 1, 2021.  If the employee receives a two-dose vaccine, the employee must also receive the second dose of the vaccine and present proof of that dose to the HR Department no later than December 1, 2021.  Employees may request a medical or religious exemption from the vaccination requirement.  Exemption request forms are available through your HR Department.  Exemptions will be granted at the discretion of HR.
